Privacy Statement
- We collect personal information from you, in cases where you fill out an enquiry form or choose to engage in therapy, including your name, contact information, and any further information you provide about yourself or your circumstances to allow us to determine whether we can offer psychological services to you. We collect your personal information in order to determine if we are able to provide psychological services that meet your needs. If you choose to engage in psychological assessment and therapy, we also collect your information to provide appropriate services including maintaining appropriate notes in line with professional standards. Providing information to us is optional.
- If you choose not to enter information regarding your reason for referral and wishes for therapy, we'll be unable to assess your suitability for therapy. If you choose to share your personal information, and engage in therapy, we keep your information safe by storing it in encrypted files with two-factor authentication and utilising Halaxy practice management software specially designed for healthcare professionals. We keep your information for 10 years at which point we securely destroy it by erasing all digital data. Our data destruction procedures occur in accordance with current regulations such as the Health (retention of health records) Regulations Act, 1996. Up to date regulations relevant to the retention and storage of health information of which health practitioners must abide by are consulted at the time of which data erasure is required. You have the right to ask for a copy of any personal information we hold about you, and to ask for it to be corrected if you think it is wrong.
